**The Role**

To work as part of an Integrated Discharge Team, completing assessments of need for individuals currently in hospital who are ready for discharge.

To work as part of a multidisciplinary team to support with discharge planning and complete timely assessments to meet eligible care needs, whist ensuring a continuous flow through the hospital and preventing delays in discharge.

**About You**

You will be familiar with working in a fast paced, high pressure hospital environment and be able to respond quickly and professionally to the demands this presents whilst maintaining service user safety.

You will be passionate about achieving positive outcomes for individuals and hold knowledge on universal services and means to provide support to individuals in a person centred way.

You will have a home first ethos and support people to return back to their homes following a hospital stay.
