We have an exciting, new opportunity for a Care Co-ordinator to join our Primary Care Network, supporting our 5 GP practices across the Braintree area to assist in re-shaping Primary Care across our communities.
The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN. The role is pivotal in ensuring all patients receive the best possible care and service.
The Care Coordinator’s role will support the clinical directors in coordinating all key activity including access to services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centered.
The role will include supporting digital initiatives and includes responsibilities for the co-ordination of the patient’s journey through primary care.

If you are a skilled Care Co-ordinator with excellent person-centered planning experience, with local knowledge of key support and advice and support to our patient population, then we'd love to hear from you.
**You will need to have the following skills / qualifications**:
- You will have experience of working within healthcare or the voluntary / community sector, supporting vulnerable groups;
- Ideally you will have a Diploma Level 2 in Health and Social Care or equivalent qualification;
- You will have strong listening and communication skills;
- You will be familiar on local resources and services and how to access them;
- You will be motivated by helping people, with care and empathy;
- In delivering person-centered support in a non-judgmental way;
- Experience of completing holistic person centered care planning assessments
