**About the Role**:
Little Waltham & Great Notley Surgery** **have a great opportunity for a care co-ordinator to join our team on a full time basis.

You will deal with patients and their carers, working closely alongside our multidisciplinary team. Being practice-based you will be able to identify patients’ individual needs ensuring they are addressed by taking into account local priorities, health inequalities and population health management risk stratification.

The main duties of the role include**:

- supporting the clinical directors in coordinating all key activity, including access to services, advice, and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
- supporting digital initiatives.
- responsibility for the co-ordination of the patients' journeys through primary care.

If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

The ideal applicant will have the following skills, experience and qualifications:

- Ideally you will have a Diploma Level 2 in Health and Social Care, or an equivalent qualification;
- Strong listening and communication skills;
- Familiar with local resources and services, including how to access them;
- Motivated by helping people, with care and empathy;
- Able to deliver person-centred support in a non-judgmental way;
