We are looking to recruit a Mental Health and Wellbeing Coach, to work within The Personality Disorder Community Team, providing 1:1 personalised support to people referred to them by team members and local agencies.

The post holder will work with a diverse range of people from different cultural and social backgrounds, with the ability to work confidently and effectively in a varied, and sometimes challenging environment is essential.

This is a vital role within the service pathway and the post holder will provide health and wellbeing coaching for some of the most vulnerable people in the community, who often have long-term conditions. They will work closely with the MDT, pathway leads and their supervisors to manage a caseload, ensuring appropriate support is available to people, families and carers.

This is achieved by empowering people with more complex needs to have greater choice and control over the care they receive, enabling choice and embedding shared decision making, so that people are informed and supported as equal partners in decisions about their care and treatment.

There are a wide range of additional weekly tasks associated with these roles, including, specific tasks within the duty team, facilitating treatment and psycho educational groups, group reflection and supervision, training, education and specific individual supervision associated with the role.

These posts will be based at our office in Bridge Street, Peterborough and Chesterton Medical Centre Cambridge, but also having the ability and flexibility to travel to our other bases throughout Cambridgeshire, Huntingdon and Wisbech.
