An exciting opportunity has arisen within North East Mental Health In-patients (Stobhill) to lead our Integrated Hospital Discharge Team. We are seeking a Discharge Coordinator to coordinating patient care specifically in relation to timely discharge from mental health inpatient care. This is a key role to ensure there is a clear focus on discharge planning and coordinating by identifying and resolving any early potential barriers through facilitating early intervention. The post holder will link in with partner organisations and third sector based providers to support the assessment of needs pre admission, on admission and during admission. They will design, execute and maintain streamlined comprehensive discharge processes whilst ensuring best practice for safe and effective discharge. You will work collaboratively with multidisciplinary teams, which include medical, nursing, Allied Health Professionals, Social Work, commissioning services, peer support workers and third party agencies by fostering positive working relationships. The post holder will use their expert mental health knowledge to influence care decisions that supports discharge coordination
across the team and in the best interests of the patient and their families, illuminating patient care at all times. The post holders requires to be a registered practitioner with the NMC, HSCP, SSSC and demonstrate relevant leadership knowledge and experience to make informed decisions. You will be educated to Degree level or equivalent relevant experience Have evidence of continuing professional development and knowledge of policies and procedures relating to hospital admission, discharge, transfers and delayed discharges.
