An exciting opportunity has arisen for a highly motivated qualified Mental Health Practitioner to join our continuing developing community team (FIRST). We are looking for individual's who have a range of experiences in Mental Health settings and are committed to supporting service users with complex needs through the secure care pathway.

The successful applicant's will have experience of working with a wide range of individuals and be able to demonstrate an understanding and awareness of the criminal justice system and its impact on individuals that use our services.
You will be able to work with other members of the team in managing an individual caseload, working within the confines of a robust care plan that meets the individual needs whilst managing any risk factors for the individual or others.

**Our Team is based a cross**: Reaside/ Ardenleigh/ Tamarind/ Elliot House
The team is recovery focused and you will be supported via a bespoke training package to work alongside service users as they maintain meaningful and purposeful lives in their local communities. We are a busy multidisciplinary team with a mixed caseload of males and female service users with complex mental health needs.
The successful applicant will be supported to develop their professional interests in line with the Team objectives.The team benefits from both regular reflective practice sessions, and shared formulations sessions alongside the mandatory clinical supervision.

Will act as an independent practitioner within a specialist community setting, with the responsibility for intensive follow up of mentally disordered offenders.

Will liaise with other professionals regarding risk management issues.

Will manage a defined caseload and provide supervision to others as appropriate.

Will manage and assess risk of violence and offending behaviour whilst being mindful of our duty to public protection.

Will be responsible for the assessment, planning, implementation, and evaluation of nursing care within agreed parameters, and have a working knowledge of local, national policies and legislation, which govern current service provision and audit.

Will be responsible for ensuring effective communication pathways exist.

Will be responsible for ensuring the promotion of equality, diversity, and rights in accordance with good practice and legislation.

Will be responsible for identifying and contributing to the training needs necessary in providing quality evidence-based care as appropriate.

Provision of a 24-hour, 7 day per week crisis intervention service to cover community-based service users within the forensic directorate.

Participate and contribute to the development of policy and strategy for community-based services within the Forensic Directorate.

Participate in liaison with non-health care professionals, i.e., Police, Probation and courts.
