**About the Role**:
We have an exciting opportunity for a Social Prescriber as Chelmsford City Health Primary Care Network (PCN) is seeking an additional part-time social prescribing link worker to join its enthusiastic team of clinicians and administrators.

The successful applicant will be working within the PCN's Social Prescribing Hub supporting patients based at the Network's four Chelmsford practices (Baddow Village, Beauchamp House, Stock and Whitley House Surgeries) and will be well supported with on-going professional development (training support, CPD and peer support).

As part of the PCN’s Social Prescribing Hub, the Social Prescribing Link Worker will provide personalised support to individuals which will enable them to improve their health and wellbeing, to live more independently and to ensure they are able to access resources within their local communities. The approach will be person-centred, with a focus on strengthening both community and personal resilience**.**

The post holder will provide support to people referred to the Social Prescribing Hub, understanding their priorities and co-producing personal support plans which will enable them to live more independently and improve their health and wellbeing.

The role will include developing relationships with key statutory, voluntary and community organisations to raise awareness of social prescribing.

The main responsibilities of the role include:

- Receiving and managing all referrals from multi-agencies and providing personalised support to vulnerable individuals.
- Working in partnership with the PCN practice teams, supporting patients and promoting positive lifestyle changes, preventing ill-health by means of behavioural change techniques.
- Completing holistic care planning assessments, analysing how needs can be met by services and other opportunities available in the community.
- Providing information and advice about a wide range of services to promote health and well-being and maintain independence within communities.
- Forging strong links with local VCSE organisations, community and neighbourhood level groups.
- Contributing to reducing demand on statutory services, hospital admissions and to combat unnecessary GP appointments.
- Working with patients in the practices or with patients in their own homes, providing personalised person-centred support.

You will need to have the following skills:

- Experience of working within the healthcare, social care or the voluntary / community sector.
- Ideally you will have a qualification in healthcare, social care or counselling.
- Experience of supporting, coaching or counselling vulnerable people, within a group setting or on a one-to-one basis.
- You will be familiar with local resources and services and how to access them.
- You will be motivated by helping people, with care and empathy and in delivering person-centred support in a non-judgmental way.
- An understanding of the needs of small volunteer-led community groups and ability to support their development.
- Familiarity of partnership/collaborative working.
- Experience and confidence in the delivery of lifestyle change interventions.
- Experience of directly supporting people with their mental health, either in a paid, unpaid or informal capacity.
- You will have strong listening and communication skills.
- Experience of individual needs assessments.
- Experience of research and data collection using tools to measure the impact of services.
- Demonstrate commitment to professional and personal development.

**About Us**:
The Chelmsford City Health PCN is made up of four separate practices which have formed a network in order to benefit from shared resources and to be able to offer healthcare services at scale to its population of approximately 42,000 patients. Two of the practices are dispensing practices and all four are training practices. The PCN practices all use the operating clinical system: SystmOne, for which training will be provided.
