**About the Role**:
Action for Children's North Gwent Family Intervention Team (Monmouthshire and Blaenau Gwent) is looking to recruit two experienced full time Family Support Practitioners to join the team of our well-established Gwent Wide Family Intervention Team Services. We are a targeted early intervention therapeutic service for children, young people and their families aged 3-16 delivering across the area of Gwent. Our aim is to reduce the development of more entrenched mental health issues. Our work is underpinned by psychological therapies including attachment, systemic psychotherapy, and behavioural approaches.
We run three Family Intervention Team Hubs across Gwent and the purpose of the services are to deliver high quality therapeutic family support which can involve one to one support with parent and child, as well as whole family sessions. The service offers needs led interventions that are focused on each family being regarded as unique. We utilise formulation and curiosity approaches to help build an understanding of the context that the family live in. We aim to empower families to help support relationships, improve communication patterns, build on resilience, and explore healthy ways to communicate feelings and emotions.
We are looking for two Family Support Practitioners in the North Gwent Family Intervention Team who will work remotely and who will be required to travel across the counties of Monmouthshire and Blaenau Gwent regularly to engage children and young people and their families in face-to-face therapeutic interventions. There will be opportunity to do some flexible home working within this role, but you will be expected to travel to meet children, young people, and their families in their homes/school and other venues accessible for them. There will be an expectation for weekly attendance at the face-to-face team meetings for reflective practice at the Abergavenny Community Centre, where the North Gwent Family Intervention Team service is based for 2 days per week. There will also be an expectation to travel to the Caerphilly Family Intervention Team for meetings when required with the wider cluster of Family Intervention Teams.
**As a Family Support Practitioner, you will be**:
- Working with a range of professionals to ensure the children and young people you support develop their full potential.
- Working with a Practice Team Leader, Clinical Psychologist and Systemic Psychotherapist to explore the needs of children and young people where emerging social, emotional, behavioural and emerging mental health issues have been identified.
- Delivering a time limited intervention up to 12 weeks in collaboration with the child or young person and their family.
- Liaising closely with your Practice Team Leader, colleagues and our partners to ensure compliance an excellence.
**You will need**:
- Strong work experience of working with children, young people and their families providing one to one and family work in the community.
- Ideally you will have experience of delivering targeted early interventions for children and young people.
- Proven record of working collaboratively with schools, social services and health partners and responding to safeguarding concerns.
- Experience in managing and holding a caseload of 10 minimum including managing risk and safeguarding.
- Experience of attending and inputting to Care and Support Plan (CASP) meetings, Child Protection (CP) meetings and Team Around the Family (TAF) meetings to meet the needs of vulnerable children and families
- Delivering time-limited interventions including one to one support and group work delivery and facilitation
- An ability to work on your own initiative
