An exciting opportunity has arisen for a Band 6 Community Practitioner to join our memory service within the Older Adults Services in Northumberland.

The multi-disciplinary team consists of a highly skilled and experienced workforce of Consultant Psychiatrists, Nurses, Psychologists, Occupational Therapists and non-medical prescribers.

Our memory service provides assessment, diagnosis and treatment initiation for adults who are experiencing memory problems.

Our community practitioners undertake initial assessments, provide feedback to multi-disciplinary team, at the point of diagnosis, we will undertake a post diagnostic review, which may include further risk assessment, support with titration or onward referral to health, social or external support services.
