**The Role**:
You will work under the supervision of registered therapy staff to assist patients to achieve their rehabilitation goals. You will also undertake baseline assessments within clearly defined parameters for the identified service, Implement, adapt and progress treatment programmes within agreed protocols and defined parameters. The use specialist technical and practical skills to deliver flexible, patient centred programmes across a variety of clinical settings. You will discharge or transition care within agreed protocols and defined parameters.

**Duties may vary depending on the care group / department or assigned area of work.**

**What we'll need you to bring**:
Qualifications and Experience
- HNC in healthcare, education or social cares or SVQ III in healthcare, education or social care or equivalent experience in a health, education or social care environment.
- Short specialist courses relevant to specific clinical area.
- Car Driver with a full, valid UK/EU/EEA licence

Knowledge and Skills
- Awareness of health legislation relevant to the clinical area.
- Knowledge of the therapeutic process, techniques and / or activities relevant to client group.
- Basic assessment skills.
- Effective listening, communication and interpersonal skills.
- Time management, planning and organisational skills.
- Working knowledge of basic information technology.
- Ability to work individually, or as part of a team
- Ability to learn and develop new skills and maintain a record of learning.

**It would be great if you also have**:

- Breadth of experience of therapy or rehabilitation /re-ablement in a health, education or social care environment

**Contract type**:
Permanent

Full time

37 hours

**Location and Working Pattern**:
This role will be based in Home Assessment Team base to be confirmed.
