**About us**

Crisis is the national charity for people experiencing homelessness. We know that homelessness is not inevitable. We know that together we can end it.

Crisis Skylight Croydon offers IAG, 1-1 coaching and practical support to people who are experiencing homelessness in Croydon and the neighbouring boroughs. We work with several partner agencies including the Homeless Health Team, mental health services and substance misuse services to ensure that our members have access to support needed to end their homelessness for good. We have now received funding from NHS Southwest London to deliver a health advocacy service in Croydon for 12 months.

**Contract**: 12 months fixed term contract.

**Location**: Crisis Skylight Croydon, 12 Surrey St, Croydon CR0 1RG(with regular outreach into community settings)

**About the role**

As Health Advocacy Worker, you will provide high-quality support to people experiencing homelessness in Croydon. You will support Skylight members as well as residents of local supported and temporary accommodation to register with the appropriate health services and attend health appointments. You will regularly visit local hostels and accompany your clients to their health appointments.

You will hold sensitive conversations about health needs, reasons why someone might have struggled to access health services in the past and what concerns they may currently have. You will provide IAG, support and encouragement to help build your clients confidence so that they continue accessing health services independently in the future when your support comes to an end.

**About you**

To be successful in this role you will have a good understanding of the challenges that people who experience homelessness might face when trying to access health services. You have experience supporting people with multiple needs including mental health, physical health and substance misuse difficulties, a history of multiple exclusion and social isolation. You have worked within a relevant sector which could include homelessness, asylum seeker or health services. You are confident working independently in the community and are willing and able to travel to meet your clients and potential clients around Croydon.
