Locality Community Hca in Dementia

The post holder will be part of a multi-disciplinary team, contributing to the effective provision of Mental Health Services to an identified caseload of service users.
The post holder will provide a range of services including practical and emotional support to individual service users as indicated by the individual care plans provided by their care co-ordinators.
The post is a Locality Community Health Care Assistant and the successful applicant will work across the 3 Dementia in Older People's Mental Health (DOPMH) teams in the North and East Locality area of Cornwall. The post holder will hold a full drivers licence and have access to their own car.
The successful applicant will be based at either one of the Bodmin, Launceston or Liskeard bases, and will cover the North and East Cornwall Locality area.

The post holder will work in partnership with medical and nursing colleagues, ensuring physical and mental health needs of people with dementia and functional mental health needs are considered holistically. You will assist qualified staff in the assessment of care, needs and the development of programmes of care, delivering aspects of care without direct supervision.
To recognise and report with accuracy to qualified staff any change in the patient’s condition or circumstances appropriate to the degree of urgency.
To provide signposting, advice, education, support to people with their mental health needs and their families
Making sure the right information and support is given at the right time to help people with mental health needs to remain in their communities for as long as possible. To help people to seek help early and understand the services that are available.
Support local communities in developing dementia friendly communities and build strong and effective working relationships with key staff in a cluster of, GP practices, mental health and primary care community teams.
