We are seeking a skilled and compassionate professional to join our Dementia and Older People's Mental Health Team. This is an exciting opportunity for both Band 5 Primary Care Dementia Practitioners and Band 6 Memory Assessment Nurses, with roles based in Cornwall.
The successful candidate will deliver high-quality memory assessments, contributing to the early diagnosis of dementia and supporting patients and families through the diagnostic journey. They will manage a community caseload, work collaboratively within a multi-disciplinary team, and provide person-centred care to enhance the lives of those living with dementia.
As a Memory Assessment Nurse, you will be responsible for delivering comprehensive assessments, developing personalized care plans, and working closely with patients, families, and healthcare professionals to ensure seamless care coordination.
In this role, you will also have the opportunity to work collaboratively with primary care professionals to ensure integrated and effective support for patients and their families. You will provide education and advice to caregivers, helping them to navigate the complexities of dementia care and empowering them to make informed decisions about their loved one's care.
