We have a fantastic opportunity for a dynamic, enthusiastic and highly motivated person to join the community adult rehabilitation service in Hillingdon as a dietetic assistant practitioner. The successful applicant will be a part of a multidisciplinary team including physiotherapists, occupational therapists, speech and language therapists, dietitians and rehab support workers. They will be expected to manage a caseload of patients referred to the service and provide specialist dietetic assistant assessment and treatment programmes to adult patients in their own homes, care homes and intermediate care unit requiring nutritional support.

It offers the opportunity to gain clinical experience in a range of dietetic fields, including nutritional support, enteral feeding, texture modified diets and wound management. Continuing professional development is actively supported via clinical supervision, yearly appraisal and in-service training.

The post holder will be supported by band 6 and 7 dietitians. The successful applicant must be able to travel effectively throughout Hillingdon and have access to a car to carry out home visits in the community.

- Assist in dietary assessment and treatment to adult residents in their own homes, nursing homes and intermediate care unit requiring food fortification advice/nutritional supplementation
- To be able to recommend food fortification strategies to meet nutritional requirements or assess compliance with oral nutrition supplements.
- To use electronic ordering system for home enteral tube feeding equipment and feeds.
- To identify clinical risks and make appropriate referral on to qualified clinical staff.
- To assist in the delivery of patient educational sessions to patient groups as directed
- To take part in multi-agency collaborative workings to promote continuity of care.
- To provide timely and informative client reports to AHPs, nursing and other staff.
- To be responsible for managing a delegated clinical caseload.
- To ensure that own work is within professional standards and clinical guidelines.
- To be responsible for complying with legal and professional confidentiality guidelines

The successful applicant will be a part of a multidisciplinary team including physiotherapists, occupational therapists, speech and language therapists, dietitians and rehab support workers. They will be expected to manage a caseload of patients referred to the service and provide specialist dietetic assistant assessment and treatment programmes to adult patients in their own homes, care homes and intermediate care unit requiring nutritional support. Your role as a dietetic assistant practitioner for CARS will be to work in an integrated way and provide a high quality, client-led and goal focused service; including health promotion and involvement with various teaching programmes and multi-professional working across the local area.
- To work within the British Dietetic Association code of professional conduct for dietetic support workers and understand Health Professions Council standards.
- To have a working knowledge of their role in meeting clinical guidelines relevant to Dietetics andto translate these guidelines into own practice.
- To have a working knowledge of the scope of their responsibilities and when advice needs to be sought from a qualified dietitian.
- To actively participate in Personal Development Review by identifying key performance objectives and continuing education needs.
- To be responsible for gaining valid, informed consent to assessment and treatment interventions, sharing of information following legal and professional guidelines and to follow these guidelines in circumstances where clients do not have capacity to provide informed consent.
- To follow and implement Trust, departmental and professional policies, procedures, guidelines and standards within own work area.
- To be responsible for maintaining detailed and accurate client treatment records and appropriate safe keeping, ensuring confidentiality.
- To be responsible for documenting own professional activity, client activity data, health promotion, training and education data in accordance with departmental standards and trust standards.
- To be responsible for effective time management, meeting deadlines and responding to fluctuating workloads and organising, planning and prioritising clinical and non-clinical workload.
- To participate in team meetings and other meetings as requested by service manager.

Education and training
- To actively participate in the Community Adult Rehabilitation Service’s in-service training programme.
- To maintain own clinical professional development as a dietetic assistant practitioner keeping abreast of new developments in practice, incorporating them into clinical practice.
