**As a Team Leader you will be and integral member of the team working both in the office and on shift to gain the knowledge you require to be successful in the role. Shifts times are between the hours of 0715 and 2130. Shifts are generally a mix of 0715-1430, 1415-2130 and 1000-1900.**

**The role will include supporting people with personal care, cleaning around the homes, completing medication and financial checks and completing thorough records for the time you are in shift.**

**You will be employed to work within both the management team and support teams to provide care and support services for adults with learning and/or physical disabilities, and/or behaviours that may challenge to enable them to live a healthy, happy and fulfilled life.**

**You will be required to deliver support to a range of ages and genders.**

**Company description**

Our supported living services across South Yorkshire aim to help those living with any form of disability to enjoy a more independent and fulfilling life, in bespoke accommodation specially adapted to their individual needs.

Our organisation is an approved and contracted framework provider of supported living services by local authorities in the locations to in which we operate services.

We work with those using our supported living to help them with a range of activities and everyday tasks, such as personal care, money management and social and leisure activities, to build confidence and social skills. As part of this service our staff team take pride in working with the people we are supporting devising personal support plans tailored to each person’s individual needs.

**Job description**

To work as leader of the team in the provision of care and support services for people with learning and/or physical disabilities, and/or behaviours that may challenge; which aims to maximise the potential of each individual in accordance with ordinary life principles.

We need Team Leaders to take charge of the accommodation in the absence of the Manager.

**SKILLS & ABILITIES REQUIRED**

Proven ability to work on own initiative and as part of a team

Knowledge and understanding of issues relating to people with learning disabilities

A commitment to a service which provides support to vulnerable individuals, and which is respectful and according to the individuals’ wishes

The ability to offer hands on support and work shifts with a variety of people we support to meet each of their individual needs

Good communication skills (written and verbal). Ability to communicate effectively with the people we support, colleagues, managers, families and external professionals.

Commitment to attend and deliver training and development

Ability to review, monitor and create support plans

Working knowledge of support planning systems

Ability to work under own initiative

Ability to manage finances and medication ensuring policies and procedures are followed at all times

Experience of monitoring and evaluating services including the completion of audits

Experience and ability to manage and supervise employees

Proven ability to make decisions and solve problems

Strong awareness and understanding of Mental Capacity and how to complete relevant assessments

Flexible, self motivated and innovative approach to working

Honesty, reliability and trustworthiness is a must

Ability to use Microsoft Office competently

**EXPERIENCE REQUIRED**

Experience of working in a multi-disciplinary setting

Experience of managing/supervising Staff

Minimum of 12 months in a similar setting, ideally with people with learning disabilities

**EDUCATION, SPECIALIST KNOWLEDGE & QUALIFICATIONS REQUIRED**

Good general education

Working knowledge of health and safety issues

NVQ Level 3 in health and social care or team leading

Knowledge of the regulatory framework and Care Quality Commission

**OTHER REQUIRED**

Enhanced DBS Check

A minimum of 2 satisfactory references including one from your current employer

A full and complete work history including any gaps of employment and the reasons behind these

**MAIN RESPONSIBILITIES**

To assist the Manager in the implementation of the policies to minimise the effects on the peoples lives we are supporting whilst keeping them safe at all times

Ensure that each resident has their needs met through effective assessment and support planning. Whilst being able to competently reviewing and monitoring support plans.

Support and participate in supporting the people in receipt of our support with social and recreational activities in line with their support plans, including day trips and holidays whilst monitoring the persons participation to ensure activity plans are adhered to.

Monitor the Keyworker’s’ progress with individual residents.

Ensure all medical needs are met, and support the people we support to advocate his/her own needs and wishes.

Facilitate and support residents in decision making. Promote choice and involvement and complete Mental
