Cwt (Cancer Waiting Times) Patient Tracker

An exciting opportunity has arisen within our enthusiastic and dedicated Cancer Services team for 2 CWT (Cancer Waiting Times) Patient Trackers on a 12 month fixed term or secondment basis. We are looking for a pro-active, motivated individual who enjoys working in a busy admin environment to support the Trust in delivering Cancer Wait Times targets. This exciting role will focus on expediting patients through the suspected cancer pathways from when the patient is first seen through to treatment.
Duties will include liaising with 2WW Booking Office and specialties to proactively track and accurately record the patient's progress through the suspected cancer pathways and expedite the patient where appropriate.

This exciting role will focus on expediting patients through the suspected cancer pathways from when the patient is first seen through to treatment.
The CWT (Cancer Waiting Times) Patient Tracker is responsible for monitoring referrals for suspected cancer to ensure that patients’ appointments and treatment are booked to national and local cancer pathway targets. Duties will include liaising with 2WW Booking Office and specialties to proactively track and accurately record the patient's progress through the suspected cancer pathways and expedite the patient where appropriate.

1. ROLE AND RESPONSIBILITIES
To work primarily independently and on own initiative with mínimal supervision to carry out the duties of the post
1. To track patients referred as suspected cancer to ensure their pathways meet agreed timescales/targets
2. Escalate patient pathways to ensure all stages (appointments, diagnostics, MDT meetings) are scheduled within the optimum timeframes
3. Maintain close communication with the MDT co-ordinators and supporting the PTL meetings
4. To communicate with colleagues in and outside of the Trust to ensure that patients are appropriately managed on their suspected cancer pathways
5. Assist in the review of office protocols and working practices where necessary.
6. Provide support with training for new staff where required, demonstrating duties to new starters.
8. Contribute to the cancer databases for patients (Somerset), updating records and contributing to the collection of data for audit and research purposes.
9. Be familiar with national Cancer Waiting Times standards and undertake training and e-learning as required
10. Ensuring attention to detail when tracking, checking patient records are correct and that the information you are inputting is accurate
11. Any other duties commensurate with the grade and in line with the requirement of the post.
