Camden Learning Disability Service and Islington Learning Disability Partnership has been awarded a 3 year grant t to focus on a project to raise cancer screening awareness and uptake among Camden and Islington's Learning Disability (LD) populations.

**this project will**:

- Commission peer-to-peer engagement from local LD experts by experience to understand patient experience and barriers to accessing screening, including the effect of intersectionality, e.g. ethnicity and disability.
- Co-produce awareness raising “myth-busting” materials, such as short films or primary care training sessions, based on the engagement findings.
- Deliver evidence-based health facilitation approaches to support:

- join-up between primary care and specialist screening services to identify, contact and engage eligible people with an LD.
- services to make reasonable adjustments before and during appointments, including intense desensitisation work.
- services to maintain improved screening participation records.
- young women aged 18-25, families and local colleges to understand and prepare for cervical screening from age 25, including reviewing HPV uptake amongst the eligible female LD population.
- Develop a menu of least restrictive alternatives/ prevention measures for patients who opt-out, or lack capacity and it’s agreed screening is in their best interests.
- Work with screening services, PCNs, GPs, and LD experts by experience to improve and evaluate reasonably adjusted screening pathways for people with LD, including meeting theAccessible Information Standardlegal duty and implementing existing PHEguidanceandsuite ofresourceson improving access.
- Better understand local inequalities and barriers faced by people with LD along the current screening pathways, including cohort identification and invitation, provision of information, access to screening, and onward referral/ treatment.
- Raise awareness of eligibility,timelinesand processes with LD groups/ families.
- Increase skills/ confidence to participate in screening, e.g. sufficient support to take bowel screening samples; access to easy read information etc.
- Enhance LD patient experience of cancer screening pathways.
- Advance data sharing and partnership working across community, primary and secondary care to identify and invite eligible LD patients for screening, make reasonable adjustments and improve uptake recording to address data gaps in NCL screening participation reporting, particularly for breast screening.
- Understand HPV vaccine uptake in the LD population in Camden and Islington.

Camden & Islington Foundation Trust and the London Boroughs of Camden and Islington are working in partnership to provide Mental Health Services. Employees from all the participating organisations work closely together and have joint management and supervision arrangements. You may, therefore, be managed by a manager who is not employed by Camden & Islington Foundation Trust.

All staff employed or seconded directly to Camden & Islington Foundation Trust (C&IFT) regardless of the band or clinical grade will be required to work within the clinical and operational structures. The job description reflects the generic level of core competencies, skills and experience required to undertake the duties that are consummate to the band or grade.

This is a split role located in the Camden Learning Disabilities Service (CLDS) and Islington Learning Disability Partnership (ILDP). CLDS and ILDP are fully integrated health and social care multi-disciplinary team for adults and teenagers with learning disabilities either living in the London Borough of Camden/Islington (LBC/LBI) or for whom Camden/Islington maintain responsibility and / or commission services out-of-borough. CLDS/ILDP are based in Local Authority premises, but maintains very strong and active links with C&IFT (eg training, professional development, overarching clinical and professional governance).

Themain duties and responsibilitiesunder primary health care facilitation are to:

- be the key contact for cancer screening and health facilitation in Camden/Islington
- promote health facilitation to individuals and groups in Camden/Islington and encourage a consistent approach within NCL
- contribute pro-actively to the work of the Camden Learning Disability Partnership Board (“Planning Together”) and also it’s equivalent, Islington Partnership Board, as well as the Health and Wellbeing Sub-Group in order to enable improvements in health facilitation, to report progress and to share information about implementing improvements;
- provide expert advice based on evidence-based practice and to keep up to date on the latest evidence for effectiveness;
- contribute to the continuing development of the joint strategic needs assessment by ensuring that needs and data and the views of people with learning disabilities are captured in the joint strategic needs assessment;
