Calderdale Intermediate Care Rehabilitation team are looking for a well organised individual with excellent communication skills to join the team.

The post holder will be responsible for a range of administration and clerical duties relating to the flow of patients into, and out of, Calderdale Intermediate care beds and specifically identified discharge to assess beds. In addition, the post holder may be asked to liaise directly with patients and their families to obtain social histories and information regarding the patient’s usual level of function to aid therapists in their treatment and intervention planning, discharging of patients and onward referrals to community service or similar.
- Patient flow into and out of IMC beds
- Admin and clerical duties i.e. equipment ordering, booking transport, stock monitoring, documentation form meetings.
- Social history taking from patient and families.

CHFT employs over 6,000 colleagues who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital (CRH) and Huddersfield Royal Infirmary (HRI) as well as in community sites, health centres and in patients’ homes.
- Attend daily transfer of care meeting maintaining key documentation.
- Coordinate and Organise the flow of patients in to, and out of, intermediate care beds and specifically identified discharge to assess beds.
- Communication between multiple services and agencies.
- Ordering equipment following delegation from Allied Health Professionals and nurses.
- Transport requests.
- Recording notes from Multi-Disciplinary Team meeting and documenting on the SystmOne patient record.
- Under the guidance of a registered therapist, complete referrals to external agencies
- Liaise directly with patients and their families to obtain social histories and information regarding the patient’s usual level of function under the guidance of a registered Allied Health Professional.
- Deliver appropriate aspects of Medical Device Training according to competence.
- Provide departmental data input support for systems such as SystmOne, EPR and local systems including and Transfer of Care Data Base.
- Provide data input support for the Medical Device data base relating to community intermediate care Therapies.
- Order and monitor departmental stock levels for walking aids and equipment required to facilitate patient rehabilitation and discharges.
- Order stationery and maintain adequate stock levels
- Order patient information leaflets/booklets.
- Support with student/new starter induction within the department
- Direct patients and family members as required.
- Act in a courteous manner at all times. Help maintain an efficient, friendly service.
- Report all hazards, incidents, and untoward occurrences to the Clinical Lead.
- Undertake any other duties, as required, in line with the grade of the post.
- Understand the need for, and maintain, patient confidentiality.
