**Job summary**

The Patient Support Care Co-ordinator will collaborate with three other Practices within Chanctonbury PCN to improve relationships between the patients and Practices.

**Main duties of the job**

This role will work closely with Practice Teams and patients to improve services across the network and ensure outstanding patient experiences.

**About us**

Chanctonbury PCN is an innovative group of four Practices with a combined list size of 49,400 patients.

The Practices are based in Henfield, Steyning, Storrington and Billingshurst. This role is to be based solely at Henfield Medical Centre.

**Job description**

**Job responsibilities**
- To be a first point of contact for patients requiring general information relating to the practice: procedures, registration, care preferences, queries on services and navigation to services
- To promote online services and sign up patients
- Displaying Health Promotion materials, MJOG campaigns, ACCU-RX messages to patients, ensuring languages are considered and supplying specific materials to the demographic
- Provide admin support to the management team.
- Process Econsultations, signposting and booking the appropriate service
- Tell patients and the public how they can get involved in their own healthcare and with local NHS services
- Contact patients who regularly do not attend (DNA) appointments, and improving DNA rates by implementing behaviour agreements
- Support clinicians to provide At Home BP and AliveCor monitoring
- Assist Henfield Medical Centre in the admin to assist with compliance with CQC KLOEs
- Manage the Practice websites and contribute to the PCN website
- Manage the Henfield Medical Centre Social Media
- Create annual practice patient surveys
- Summarising of patient notes
- Support the Administration Department as necessary

**Person Specification**

**Qualifications**

**Essential**
- GCSE grade A to C in English and Maths.

**Experience**

**Essential**
- Customer service skills
- Sensitivity and understanding
- Excellent verbal communication skills
- Active listening skills
- The ability to work well under pressure
- To be thorough and pay attention to detail
- Competent in the use of Facebook
- Knowledge of website editing

**Desirable**
- Experience in working within the NHS
- Knowledge of CQC and policy writing

Pay: £12.86-£13.50 per hour

Expected hours: 37.5 per week
