Mental Health Practitioner for Major Trauma Service

As part of the major trauma service team, you will contribute to the assessment and treatment pathway, working as part of a developing Multidisciplinary team, working Monday to Friday 9-5 based at Cambridge University Hospital.

You will complete mental health triage, mental health assessment and brief interventions for patients referred to the major trauma team.

To assist in the delivery of a high-quality mental health service, in keeping with the visions and values of the Cambridgeshire and Peterborough NHS Foundation Trust.

Providing and undertaking timely Biopsychosocial assessments and risk management plans in accordance with CPFT’s Clinical Risk Assessment and Management policy for adults aged 17 and over, who have been referred from the major trauma unit.

Promoting a safe and effective service throughout the team, giving expert advice and support to all referring clinicians.

Maintaining a visible, professional presence where patients, carers and staff can turn for assistance, advice and support.

As part of the team, the post-holder will contribute to the multi-disciplinary clinical assessment and management of patients referred to the service in collaboration with other team members.

To participate in and promote close links with local mental health services and other agencies to develop effective care plans.

To contribute to the research component of the programme, supporting data collection.

To conduct Biopsychosocial and risk assessments for individuals that have been referred via the major trauma centre

Undertake screening of patients admitted to the dedicated Major Trauma Ward and outlier wards using the Post Traumatic Adjustment Scale (PAS).

Provide a high level of clinical expertise in the assertive management and assessment and care of patients presenting with mental health problems and psychological need.

Ensure that care is based on the assessment of care needs, and includes a thorough risk assessment and risk management plan.

Provide post assessment brief intervention support - agreeing and reviewing of mental health treatment and management plans, monitoring progress and making adaptations where necessary. Ensure any plans are patient centred.

Refer to appropriate mental health in-patient services - psychological and psychiatry liaison services, Drug and alcohol team etc.

Ensure good communication with other members of the MTC and wider psychological medicine service as well as CUH hospital staff about changes in care plans.

To liaise with other agencies caring for patients, including, Psychological wellbeing services, community mental health teams and GPs to provide clear plans for all patients.

Manage and coordinate an in-patient caseload.

Work with the Major Trauma Service / Major Trauma Ward MDT to meet the mental health needs of the patient.

Provide education to relatives to help support patients

Support CUH discharge planning teams in relation to appropriate advice and facilitation to onward mental health facilities as required.

Provide advice and guidance to MTC coordinators who identify possible mental health needs in post discharge follow-ups.

Where applicable, input into the Major Trauma Rehabilitation Prescription in relation to mental health management plans / needs.

Utilise recent research findings in the advancement of best practice and in the delivery of an evidenced based service.

Engage in psychological medicine Service educational and training events to fulfil requirements of clinical governance.

To provide support and expert advice to colleagues involved in the care of patients with mental health problems and to develop systems to ensure that this support is available in a range of ways.

Ensure that the requirements of the Mental Health Act 1983 and other statutory regulations are observed and implemented within a medical setting.
