Job Introduction
A new exciting opportunity has arisen within the Adults with Learning Disabilities and Autism Team for an Adult Practitioner.

We work with those with an LD and/or Autism across Telford and whilst we are an adults team, our Transition workers are involved with those coming through that process from aged 14.

You will be expected to work in a person centred way to enable adults who are Autistic and/or diagnosed with a Learning Disability, to reach their full potential, maximise their social inclusion and achieve positive outcomes.
