The Adult Community Service is an integrated healthcare team providing long-term conditions management, rehabilitation & community nursing services in Mansfield/Ashfield.

We are looking for Nursing Associates to join our dedicated Community Nursing teams who are committed and passionate about promoting health, well-being and independence. We are looking for a dedicated and enthusiastic clinician who believes in the provision of patient-centred care.

A key aim of our service is to reduce unplanned admissions and facilitate discharges from the acute care environment. The team members include, Community Nurses, District Nurses, Specialist Nurses, Support Workers and Social Workers, also working closely with GPs and the voluntary sector.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
