Clinical Support Worker

Main Responsibilities Care To work under supervision ensuring the provision of high quality care and practice; in accordance with Derian House Childrens Hospice policies, standard operating procedures and guidelines for designated babies, children or young people. To accurately monitor and document the care for the children and young people and ensure that the care provided adheres to individual care plans. Ensure that the children and young peoples daily routines and individualised needs are respected at all times. To be responsible for monitoring, reporting and recording any changes in the condition of the babies, children or young people being cared for, including health, emotional and social care needs.
To be responsible for administering medicines and pharmaceutical products to children & young people following Derian House Childrens Hospice policy, standard operating procedures and NMC standards as directed and supervised by registered staff. To be competent in Derian Houses defined core clinical competencies and individual care needs as a Clinical Support Worker. To comply with manual handling policies procedures in the care and treatment of the children and young people. Recognising the scope of clinical competence at all times.
Work as part of the team contributing to the efficient running of Derian House. This includes taking responsibility for the care of the babies, children and young people and any tasks as delegated by the team leader. Foster a culture of respect and consideration between all team members. Respecting the strengths, skills and knowledge of others as well as sharing own knowledge and skills.
Ensure the respect, dignity and rights of babies, children, young people and their families are upheld at all times. Act at all times in accordance with relevant legal, ethical and professional legislation and frameworks. To assist, as requested and appropriate with the clinical audits to enhance the delivery of care. Contribute positively to maintaining and implementing the required clinical standards.
To be responsible and adopt a positive approach in the process of preventing and controlling infections. To maintain appropriate clinical records ensuring that they are stored securely and confidentially. Agree regular objectives with supervisor and demonstrate progress. Quality Assurance Contribute to the management of quality assurance issues reporting and recording incidents, responding to complaints and feedback in line with Derian House policy and standard operating procedures.
Participate in the formation of risk assessments for babies, children and young people in your care, reporting and recording all incidents, accidents and near misses, actively participating in any corrective action required to minimise future risk. Adhere to the prevention and control of infections code of practice (Health & Social care Act 2008) and Derian House Childrens Hospices policies and procedures to prevent infection. Contribute to the safeguarding of babies, children & young people ensuring practice complies with Derian House Childrens Hospices and identifying any areas of concern and reporting them in a timely way to the Social Worker or a senior member of the care management team. Communication Regularly attend team and other meetings as appropriate, contributing positively to discussions and debates and participating in shared decision making.
Ensure effective communication including the dissemination of information accurately and timely to support effective team working. Work collaboratively within multi-disciplinary/multi-agency teams both internally and externally to Derian House through the establishment of effective communication systems to support the delivery of high quality care. Act at all times as an ambassador for the organisation and maintain positive and effective communication both internally and externally in representing the service offered by the hospice. Produce clear, legible and consistent written documentation in accordance with professional standards, e.g., NMC and regulation bodies.
Adopt and present a proactive approach to personal and professional development, participating in supervision, annual appraisal, learning opportunities and reflective practice. Under the supervision of registered staff support the education and professional development of colleagues and students. To maintain confidentiality in all aspects of the role, notably information regarding the children and their families, staff and the organisation itself. Disclosure to any unauthorized person is considered a disciplinary offence.
Health and Safety To be responsible and assist the registered staff in the process of preventing and controlling infections. Ensure all equipment is utilised and stored safely and ready for use to support the needs of children & young people whilst at Derian House.
