MAIN DUTIES AND RESPONSIBILITIES Improve adoption and/or optimisation of new technology to enhance patient access and experience and increase PCN productivity Build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients (including shared appointments between practices to aid delivery of enhanced access) Lead an improvement approach to change including building capability for quality improvement within the PCN and system wide approaches to problem solving Review and improve the PCNs digital maturity Use data, and improve data quality, to understand demand, capacity and activity and drive improvements in patient experience of access, operational efficiency including better matching capacity to need, as well as staff experience at work Use data to support population health management, support understanding of the type and intensity of support/training needs of the PCN and coordinate this support, including through organisational development programmes Facilitate clinically led innovation and the effective adoption of improvement initiatives, including integrated working at neighbourhood and/or place level to improve access to services for patients. Work as part of the PCN management team using to provide technical support for the delivery of PCN priority projects and delivery of the DES contract, including the Impact and Investment Fund framework. Create and run custom reports and templates at PCN practice request Report at practice and PCN level for both practices, relating to CQRS and QoF based queries Provide technical support to set up shared clinics e.g., for Enhanced Access Support staff with IT set up and training on appointment and new GP IT initiatives Be the first point of contact for queries relating to technology (infrastructure, systems, and software), providing a timely and efficient response Assist in ideas and initiatives regarding digital improvements Develop and maintain websites where required Develop a PCN social media strategy COLLABORATION: To develop relationships and a collaborative working approach within the practices and PCN, communicating effectively with colleagues, practice staff, wider PCN partners, and patients and carers where applicable; recognising the need for a variety of communication methods; being confident and competent at interacting at all levels To provide learning opportunities for the whole multi-professional team within primary care To develop and evaluate more effective and streamlined systems and services Use professional judgement, to take responsibility for making and justifying decisions in unpredictable situations, including in the context of incomplete/contradictory information Uphold high levels of professionalism and always abide by Caldicott and GDPR confidentiality guidance, across all working scenarios.
