Patient Flow Co-ordinator

The role of the Patient Flow Co-ordinator will be to support the Care Closer to home Team in the delivery of the complexity hub service to adults with complex health needs within Waltham Forest.
The post holder will have previous experience of working in an administration role and providing care co-ordination to patients. You will be expected to support the team, department and organisation to achieve the Trust’s Values in their day to day work. These are the 5P’s:

This Patient Flow Co-ordinator will have overall responsibility for the coordination of the MDT for patient's with complex needs. The smooth running of the MDT is key to the projects success and this role is crucial in reducing the amount of time patients are spend in acute hospital care and providing support close to home.
You should have well-developed communication skills with patients/ colleagues and be able to provide an initial response to patient and other agencies queries, concerns and sign post/direct as appropriate. You will also be able to prioritise your work, be a team player, use your initiative and meet deadlines. You be able to manage your own time and deliver on the Key objectives of the project.
The project will initially run for 9 months.

Probationary Period
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement - London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).
