We are looking for individuals that enjoy working with people, primarily in the community, having a positive impact in supporting rough sleepers from street into accommodation.

You will be responsible for delivering intensive case work to those with a range of complex needs, including health, substance use, mental health and immigration issues. This is a community based role working with rough sleepers and vulnerably housed people in Ealing with the aim of finding lasting accommodation and rebuilding lives as part of a supportive and experienced team.

In this role you will:

- Undertake early morning and late evening street outreach shifts to locate new and existing rough sleepers. Devise a plan to exit street homelessness as a matter of urgency with the service user and see this through to its successful outcome.
- Assess new clients face to face in a community setting to establish personal needs and circumstance that will form the basis of a support plan.
- Compose, deliver, and be held accountable for the production and distribution of information in the form of Daily Reports, data collection and accurate running records.
- Implement short to medium term tenancy support in property immediately after accommodating service user and refer into longer term support where appropriate. This is to minimise the risk of a tenancy failing in its infancy and the service user returning to the streets.
- Empower service users to contribute to their own support plan by using expert knowledge of legislation and locally available services to identify realistic, achievable outcomes.
- Identify and manage risk presented to self, colleagues and service users using appropriate preventative and recording tools to identify and reduce possible future risk. Adhere to lone working procedures.
- For this role staff can claim an additional 20% pay premium for hours worked between 8pm and 8am Monday to Friday.

**About you**

Some experience of working with vulnerable people with complex needs and supporting them to achieve and sustain positive outcomes would be beneficial.

To succeed in this role, you will;
- Take a proactive and empathetic approach in supporting some of the most vulnerable members of our community.
- Ensure you are flexible in how you support our clients.
- Work well as part of a team collectively working to achieve a shared goal.
- A full UK or equivalent driving license is required for this role.
