**Social Prescriber for Children, Families and Safeguarding**

**with Kendal Primary Care Network**

**Overview**

The 3 GP Practices of Kendal PCN are looking to recruit an experienced, organised, compassionate person to help their teams meet the Safeguarding needs of their patients, provide support to vulnerable children and families, and deliver excellent Safeguarding practice.

The role will be both administrative and patient facing. It is a new and developing role.

The post holder will work across the 3 GP Practices providing central co-ordination for safeguarding information, communication, and care planning. You will help to implement and update Safeguarding policies.

You will also be part of a team supporting young people and families where there are Safeguarding concerns, emotional, behavioural and relationship difficulties.

You will be a key contact for liaison with external services involved with Child Safeguarding and will attend MDT meetings and Child Protection/ Safeguarding meetings.

You will be assisted by Practice Safeguarding leads and Supervised/ Line Managed by Kendal ICC

Clinical Lead (GP). You will be part of our diverse Primary Care network team and be supported by our other well established Social Prescribers.

**Hours**:

- 37.5 h per week typically Monday to Friday with occasional weekend commitments (part
- time/job share considered)

**Salary**:

- £27,000-£31,000 annually depending on experience

**Base
- The Ferguson Centre, The Gillinggate Centre Kendal, operating within the Kendal Locality**

**Key relationships inside Kendal PCN- Practice Safeguarding leads in the 3 GP surgeries, Kendal ICC**

Clinical Lead GP, Social Prescribers, Mental Health practitioners, Clinical Director, PCN Manager

**Key relationships outside Kendal PCN- Community midwives, Health Visitors, Childrens Social Care ** and Safeguarding Hub, Strengthening Families team, Schools, Childcare settings, Early Help teams,

MARAC teams, ICB Safeguarding team

**Main Responsibilities**

**Administrative**:

- Oversee the workflow of safeguarding reports and documents and ensure they are circulated to appropriate clinicians in line with practice safeguarding polices.
- Summarise and code the Safeguarding documents onto patient records highlighting to clinicians any significant information.
- Maintain a register of children on Child Protection Plans, Child in Need plans and other children for who there are Safeguarding concerns, for each of the 3 surgeries.
- Ensure correct codes and alerts are added to patient records to identify children and households where there are Child protection and safeguarding concerns.
- To assist with data collection and audit in relation to safeguarding, to support and improve standards for safeguarding practice across the PCN.

**Safeguarding Partnership Working**:

- To prioritise and respond to Child Protection meeting invitations and prepare appropriate documentation with the assistance of GPs.
- To attend Child Protection, Early Help and other MDT meetings to represent the GPs.
- To implement Child Not Brought Primary Care policy with regards to missed clinical appointments.
- To follow up on children who have not had their routine immunisations as advised by the

Nursing teams.
- To represent the GP teams and coordinate communication and meetings with third parties eg. Social workers, Safeguarding hub, Health Visitors, Schools, Strengthening Families team.

This includes making notes and updating the patient record with shared information and agreed actions.
- Where there is overlap and appropriate skills/ experience work with adult safeguarding issues, eg Domestic Abuse and other issues affecting parenting/caring abilities.

**Expert Practice and Development**
- Be a role model for all clinical staff by practicing safeguarding to the highest standards whilst having a high degree of professional autonomy.
- Be able to sign post staff to relevant agencies and assistance for all types of safeguarding and support for vulnerable families.
- Be aware of local resources and disseminate updates to colleagues.
- To support with case reviews and dissemination of learning from these to colleagues.
- Update and develop policies and systems, guidance, and training to support excellent

Safeguarding.
- Demonstrate commitment to developing/ maintaining own expertise and receiving regular support and supervision.

**Direct work with Children and Families**
- As part of our Relationship health team, after training, carry out holistic assessments of children and their parents/ carers emotional health, stress levels and their relationships.
