Bromsgrove PCN Frailty Team ADVANCED CLINICAL PRACTITIONER BAND 8A 37.5 HOURS PER WEEK Hours between: 08.00- 17.00 Monday to Friday. We are seeking Advanced Nurse Practitioner roles to work with local care homes to provide a high standard of care utilising their advanced clinical and teaching skills. The role will be based with the Bromsgrove Primary Care Network and is a role employed by Herefordshire & Worcestershire NHS Health & Care Trust on behalf of the PCN.This is an exciting opportunity to drive local health improvements and build effective working relationships with local care homes. You will be supported by other ACPs, Senior Nurses, Clinical Pharmacists, and Lead GPs and working seamlessly with core Community Services. The induction programme will help to create opportunities to build necessary expertise and experience for this role.We are looking for experienced clinicians with excellent advanced health clinical skills who are passionate to make a difference for the local population.
