Our Organisation is looking for a Care Coordinator to join our client's healthcare team. As a care coordinator, you will be responsible for managing elderly and disabled patient's care, overseeing treatment plans, collaborating with other health care professionals.

**Responsibilities**:

- Organise the care team, training, and schedules
- Ensure staff are aware for health and well-being policies
- Managing rotas/schedules. Keeping accurate records.
- Maintaining good working relationships with customers and Care Workers.
- Understand individual's needs and treatment plans
- Ensure patients have access to high-quality health information
- Coordinate multidisciplinary teams
- Maintain patient confidentiality and quality care
- Provide care to patients if required or to cover shifts.
- Skillset of a Health Care Professional

Skills and requirements
- Appropriate trainings
- Excellent organisational skills
- Decision making and planning skills
- Relevant work experience
- Awareness of patient safeguarding process

Essential:
Own Car with Driving License

Happy to move to Shepton Mallet

Job type : Full time

**Salary**: £24,000-£28,000

**Job Types**: Full-time, Permanent

Schedule:

- 10 hour shift
- 12 hour shift
- Day shift
- Monday to Friday

Ability to commute/relocate:

- Shepton Mallet: reliably commute or plan to relocate before starting work (required)

Licence/Certification:

- Do you have a valid Driving License and own a Car? (required)

Work Location: In person
