Bournemouth, United Kingdom Somerset County Council Full time

**Why should I choose Somerset Council?**
Good question.
Well, for a start, we’re a hugely ambitious council, committed to improving lives for the residents, communities and businesses of Somerset.
And that’s why we’re working so hard to build a sustainable organisation with a culture that rewards, values and recognises our staff, and gives everyone here the opportunity to grow and develop, personally and professionally.
If you’re looking for a place where you work truly matters - where YOU truly matter - we might be just right for you.
We also promote a healthy work-life balance and offer flexible working arrangements wherever possible, including working from home.

**What will I be doing?**
'Having the opportunity to positively impact another's life is a gift. To do this every day, what a life'
Looking for your first step into management in your health and social care career? Could you lead a team of Social Workers within a health setting while helping to find rapid solutions to support people that do not need acute hospital care?
We're looking for experienced practitioners who are already working at an advanced level who want to further develop their practice and professional leadership in teams. With a number of Advanced Practitioner posts across the county, we’ll suitably match you to the right team, giving you the opportunity to excel in what you do best.
You’ll work with **Yeovil District Hospital and Musgrove Park Hospital.**
In return, we'll support your development with leadership and management training.
We know this profession is challenging and some days are tougher than others but we are here for you, for each other and the people we serve. We work together to achieve the best possible outcomes which makes this one of the most rewarding careers out there.
Not currently living in Somerset? **Subject to meeting qualifying conditions, we offer relocation of up to £8,000** to use towards the cost of relocating. With landscapes of outstanding beauty, world heritage and medieval sites steeped in history, beaches, cliff top walks, local produce and festivals, there’s something for everyone
**Role Overview**:

- Have managerial and performance oversight of a team of staff who will be tailoring solutions around individuals’ needs when they no longer require an acute hospital admission by working with the individual, their families and our health and community partners
- Proactively supporting timely discharge processes by working with Discharge Liaison Services
- Maintain pace and focus on the individual and their discharge.
- Be able to use performance and hospital related data to prioritise resource as and when required to accommodate periods of escalation.
- Utilise the intermediate care model to assess services with NHS and care providers on a locality basis so that individuals remain in their own homes wherever possible
- Under direction from the Service Manager provide social care challenge to health colleagues regarding decision making, planning and legal requirements.
- Work closely with community, health and the third sector, through the neighbourhood hubs to provide better outcomes for people, develop plans and reduce the need for readmission
- Liaising with key partners to facilitate improved provision of services, team training and recruitment
- Supporting admission avoidance work

**What kind of experience or qualifications do I need?**
To join us, you will need to:

- **Be a qualified and registered Social Worker**:

- An experienced qualified social work practitioner with significant post qualification experience
- A driver with your own transport
- An inspirational leader
- Have a strong commitment to promoting relational and strengths-based practice
- Possess intellectual rigour alongside creativity
- Be self-aware, able to critically reflect, and demonstrate integrity, empathy and compassion
- Have strengths in listening, speaking out and advocating for equality and social justice

We’re proud to be here for the people of Somerset. And that means everyone in Somerset. An important part of this is ensuring that we are as diverse and inclusive as the people and communities we serve.

**What will I be offered in return**?
We are going through a period of transformation and now couldn’t be a better time to join our service. Part of the Somerset resetting process is to strengthen and further develop our learning and development offer. An exciting new programme of training to promote the knowledge, skills and qualities we need for the future in social work will be coming soon.
