North Bournemouth Primary Care Network are looking for an enthusiastic, dynamic, and experienced Advanced Practitioner to join our expanding team of healthcare professionals supporting our Practices which currently include clinical pharmacists, First Contact Physios, Mental Health Practitioners, Social Prescribers, an advanced care home team and an anticipatory care team.

**The position would be for 30 hours a week, working Monday to Friday, 1.30pm to 8pm - Salary is up to Band 8a depending on experience.**

As a member of our ARRS team, you will play a vital role in the delivery of care to patients across our member Practices. Working as part of the PCN team, you will help practices with “on the day” requests which will include online & telephone triage, face-to-face consultations, daily triage clinics and visiting patients in their home. You will provide care for patients from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

**About Us**:
We are a friendly and supportive PCN made up of 4 Practices, Banks and Bearwood Medical Practice, Kinson Road Medical Centre, Talbot Medical Centre and Village Surgery. We cover a patient population of approximately 58,500. Our Practices use SystmOne clinical system.

**Responsible for**:Providing a high standard of patient care, as part of a dedicated team, in an efficient and confidential manner whilst working within professional boundaries and qualifications.

**Job Summary**:The Postholder will be required to work as part of the PCN team, helping practices with “on the day” requests which will include online & telephone triage, face-to-face consultations, daily triage clinics and visiting patients in their home. The Postholder will work with the PCN Practices, supporting their duty teams to make referrals and to meet the needs of patients effectively and efficiently.

The Postholder will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

**Core tasks**:
**1. Clinical**

To assist in the online and telephone triage of “on the day” patient requests.

To undertake home visits, assessing and treating elderly/frail/housebound patients within the boundaries of professional qualifications.

To work autonomously, but with supervision from a GP, to advise on or signpost patients to self-care, to arrange home visits, or to arrange for face-to-face consultations to either see directly or another member of the clinical team.

To assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis on an acute basis within the boundaries of competence.

To clinically examine and assess patient needs from a physiological and psychological perspective, interpret test results, and plan clinical care accordingly.

To arrange investigations as appropriate with other members of the practice team or with acute care and community services as appropriate.

To work within the clinical team to treat patient needs in accordance with evidence-based practice and national and practice protocols within level of competency, seeking the advice of the duty doctor or other staff if outside level of knowledge.

To discuss with the duty doctor, the needs of patients presenting with complex health needs or conditions requiring treatment, hospital admission or referral.

To liaise with secondary care and community teams as appropriate to ensure the immediate care and treatment needs of patients are met as appropriate.

To manage inbound administration (post/electronic communications) to ensure any medical intervention required is referred to duty GP for appropriate action.

**2. Teamwork**

To work autonomously, as part of a busy team to manage and triage requests for “on the day” services, whilst also being flexible to support the wider PCN team.

To act as a clinical role-model/advocate, working alongside the PCN team to develop and deliver care that is responsive to changing requirements.

To support and educate patients in health wellbeing and prevention.

To be an active and integral member of the PCN team, attending meetings and training sessions, as required.

To contribute to policy development and implementation of procedures and guidelines in relation to own area of work and that of the PCN team.

To assist in maintaining a safe environment for the PCN team, patients, and co-workers, ensuring that all statutory and local regulations (e.g. Health & Safety, COSHH, clinical waste management, infection control) are implemented and followed

**3. Administration**

To ensure data is recorded accurately and timely onto patient records using SystmOne and other IT software, with due regard for record management and patient confidentiality, storing
