We are seeking individuals who a Registered Nurse or an allied HCP with experience of undertaking assessments.

As part of the clinical role, you will support patients and their families through the CHC process.

You will be committed to delivering an excellent service where the people are passionate about doing things differently and working together to grow a high performing, flexible organisation that has the customer at the heart of it.

The role includes both remote and face to face assessments alongside office attendance on some occasions. Therefore, the post holder should hold a full UK driving licence and have access to a vehicle to be able to fulfil the requirements of this post.

As a Senior Practitioner you will be using your clinical skills and knowledge to assess patients using the National Framework for Continuing Healthcare. You should have knowledge of specialist areas acquired through experience and training, plus further specialised knowledge of the personalised healthcare commissioning process. Knowledge and experience of working with the National Framework for NHS Continuing Healthcare and Funded Nursing Care is essential.

You would be expected to attend multidisciplinary meetings and communicate complex issues to a wide range of health and social care professionals, patients, carers and family members. Therefore, excellent communications skills, both verbal and written, are essential.

You would need to be an experienced negotiator, pro-active and have a positive approach to problem solving. The ability to analyse complex and often conflicting information from various sources, to aid decision making, is imperative, as is the ability to work under pressure, to meet tight and frequently changing deadlines.

- Working within a team of Personalised Healthcare Assessors, taking a lead role in complex cases, ensuring a high-quality service is delivered, supporting and mentoring staff to achieve continuous improvement
