A fantastic job opportunity has become available for a care co-ordinator to join our Home Care Team at Moorlands Care Moorlands Care is a family run business that prides itself on delivering person-centred care in the Darlington community.

**Main Duties**:

- Organise and co-ordinate the weekly rotas and care service.
- Dealing with queries from clients and their families, liaising with other health care professionals as and when needed.
- Supporting management with business development.
- Carrying out pre-assessments for new clients and developing person centred care plans.
- Working in the community
- delivering care to our clients including working evenings and every other weekend.
- Completing staff supervisions and observations
- Promote safe working practices.
- Participate in on call rota.

**Required education and skills**:

- NVQ level 3 in health and social care (Or equivalent)
- Previous experience working in the Health and Social care setting.
- Full driving licence.
- Flexible.
- IT skills.
- Excellent communication skills.
- The ability to work under pressure.

This is a full time 30-40-hour permanent position, within a fantastic company that encourages staff to reach their full potential. Continuous training and support will be offered.
