Service Coordinator

'To Coordinate all functions within our Dementia Service.
**Working Monday
- Wednesday 9am -4pm**
1. To be responsible for booking all venues ensuring they are within budget, have good access and are suitable for group events.
2. To be the first point of contact for referrals
3. Complete basic assessment form via telephone ensuring that we are data compliant and update our internal databases
4. Pass assessment form to the service manager to check eligibility on NHS system
5. Create groups on our internal database in conjunction with the Health & Wellbeing Coordinators
6. Add clients to the individual groups on our database and create waiting lists where appropriate
7. Liaise with Health & Wellbeing Officer with regards to taking payments for class attendance for our memory club classes
8. Prepare register of attendees and pass to relevant facilitators/ Assistant each month
9. Prepare feedback forms and relevant monitoring forms and pass to relevant facilitator at the beginning of each course
10. Prepare resources/activities for classes and liaise with facilitators and assistants
11. Notify facilitators of any cancellations
12. Notify clients of any changes to activity schedules
13. Update venue gantt chart
14. Advise clients/ carers of class start date/ times and venues
15. Arrange taster sessions for paid for services if appropriate
16. Complete relevant training to allow for cover for facilitator absence
17. Liaise with the service manager regarding non
- attendance
18. Liaise with Office Manager regarding supplies required for each group
19. Ensure that classes for CST are well attended with a minimum of 6 clients per class
20. Check eligibility for CST
21. Identify new venues ensuring they are within budget and cost effective
22. Research and create a resources folder on the shared drive so facilitators can access resources and information.
23. Ensure that the coordinator provides facilitators with relevant resources to run their groups.
24. Liaise with clients and or carers regarding non
- attendance.
25. Attend meetings where necessary
Ensure service delivery is in line with relevant quality standards.

Previous experience of working with older people
Previous experience of working with older people with dementia
Previous experience of facilitating group activity
