**Duties**:

- Assess the needs of clients and develop personalized care plans
- Provide guidance and support to senior carers, carers and other healthcare professionals
- Conduct regular assessments and reassessments to evaluate client progress
- Collaborate with healthcare providers, families, and clients to ensure effective communication and continuity of care
- Maintain accurate and up-to-date client records
- Provide supervision to carers in the service

**Requirements**:

- Proven experience in a senior role or a similar role
- Strong knowledge of working in the care field
- Excellent communication and interpersonal skills
- Ability to assess client needs and develop effective care plans
- Proficient in using electronic health record systems
- Compassionate, patient-centered approach to care

**Salary**: £25,000.00-£26,000.00 per year

Expected hours: 40 per week
