**Salary**: NJC points 17 - 20: starting salary, £26,357 (full-time equivalent), actual salary, £21,085 (29.6 HPW)

**Function of the post**:
To work particularly, but not exclusively, with young adult carers aged 16-25, providing information, advice and support using a person-centred and outcomes-focused approach. To enable and empower young adult carers to identify priorities, plan actions and achieve positive changes in their lives.

To work proactively with relevant organisations in Newcastle to identify, inform and support young carers.

To contribute to the effective and efficient operation of Newcastle Carers and support the Chief
Executive and staff team in the delivery of the organisation’s aims and objectives. The post-holder will ensure the highest standards of service delivery in accordance with policies, ensuring that the service is appropriate, sensitive and accessible.

**Key Responsibilities and Duties**:
1. To undertake transition assessments. (Training will be provided).
2. To provide information, advice and support to young adult carers, including targeted support through transitions, and signposting and referring to other appropriate internal and external support.

3. To provide one-to-one carer support. Assist carers to decide their priorities, create an action plan and then support them to make changes they want in order to bring about long-term change.

4. To work with and provide appropriate information, advice and support to young carers and their families to identify and address individual and family needs and any inappropriate caring tasks, and to empower them to achieve positive outcomes.

5. To provide informal advocacy for young carers/young adult carers through attending relevant health and social care, education/training meetings as appropriate, to ensure their needs are considered.

6. To carry a personal caseload and maintain records in accordance with Newcastle Carers requirements, including report writing, carer records, correspondence and monitoring and evaluation systems.

7. To support the facilitation of group sessions for young adult carers and young carers, offering information, activities and peer support.

9. Working with communications colleagues, to create and maintain online information resources for young adult carers and young carers, e.g. via Facebook, website information, links to national web-based resources etc.

10. To work with organisations such as schools, colleges, youth groups, Job Centre Plus, GP practices, Children’s and Adult’s Social Care, and voluntary sector organisations to raise awareness of the needs of young adult carers and young carers, promote good practice and encourage referrals.

11. To support promotion and awareness raising of the needs of young adult carers and young carers in Newcastle, for example, within Strategic partnerships.

13. To organise events and activities for and with carers that promote the aims and objects of Newcastle Carers.

14. To ensure that decisions or judgements which entail a high level of risk are referred to the Chief Executive.

**Person Specification**

**Knowledge, Experience and Skills Essential Desirable**

A minimum of 2 years’ experience of working with young
1. people, children and their families in challenging circumstances.

NVQ Level 3 in Health and Social Care (or equivalent
2. relevant qualification or experience).

Experience of facilitating/running group sessions for
3. young people.

4. Experience of providing confidential, independent information, advice and support to individuals.

Experience of managing a varied and challenging
5. workload, including carrying a personal caseload.

Understand the issues that affect young adult carers/young carers and their families, the impact this

6. has on them and the legislation and policies that impact their lives.

Excellent all-round communication skills with the ability to develop and maintain constructive relationships with
