**Job Details**

A great opportunity to join Suffolk County Council's West Area Teams as a Senior Practitioner working from either **Bury St Edmunds or Sudbury** with **hybrid working** options including some working from home.

**Your role**

You will provide professional support to practitioners within the team alongside wider management support to include the delivery of integrated Care and Health Services. You will be responsible for the delivery of the council’s strategic and visionary agenda to provide effective and best value services for the people of Suffolk. You will be expected to promote Suffolk County Councils vision, objectives and priorities effectively to your staff, our partners and the public. This includes inspiring others as a role model and demonstrating the values underpinning the organisation.

**Your responsibilities**

You will provide senior support as part of a team alongside a degree of management cover to support the team manager. This will include the ability to work within the Integrated Health System and representation of Adult Social Care.

You will be responsible for providing effective supervision through promoting a Signs of Safety approach to supervision and case management.

As part of the role, you will be expected to promote the values of Suffolk County Council and demonstrate professional leadership and promote development within the team. This includes promoting best practice, Organisational values and promoting a culture of high quality and best value for people living in Suffolk.

As part of this role, alongside supporting practitioners with case management and good practice, you will be responsible for managing complex and challenging cases.

**You will need**:

- to promote a high-performance culture within the area teams that is flexible, innovative, responsive to changing priorities and empowers people to deliver excellence, best value and continuous improvement for the people of Suffolk
- a clear focus on customer, families, carer and community needs, in the context of demand reduction, ensuring preventative and long term sustainable solutions which enable people to live long and fulfilling lives by maximising their independence
- to support the Team Manager in the Leadership and management of the area teams, ensuring effective operations in planning, programme development, human resources and staff development, and financial oversight.

**The team**

You will provide professional support to an area teams in a particular geographical area of Suffolk, as we progress to the emerging new operating model and locality based alliances. The area that you work in will have a number of area teams, and will align to the clinical commissioning group footprint. You will support the Team Manager and the Senior Management Team, who will be responsible for the operational delivery within it. You will form part of the Integrated Neighbourhood Teams, and provide support and advice to frontline staff delivering social work services, occupational therapy, reablement services, adult mental health and learning disability services. You will be expected to hold a small caseload to ensure that you maintain your professional focus.

You will work with local Contract and Service Development Teams and managers to understand, develop and share good practice, ensuring frontline teams share information to inform future commissioning models and new ways of working.
