**Job overview**:
We have an exciting opportunity within Adult Social Care for an administrator to join our dedicated admin team in the Acute setting currently based on the Salford Royal Hospital site.

You will be supporting a team of social work practitioners in enabling the safe discharge of patients from hospital back to a community environment.

**Main duties of the job**:
The role involves working in the social work office answering a variety of telephone calls and queries from professionals and members of the public, inputting and processing referrals into client based systems and liaising with community district nursing to facilitate seamless clinical support to patients discharged home who will require ongoing treatment.
