The Community Access team are the ‘front door’ into adult social care, they give Information advice, guidance and support on care and support for adults. They carry out initial assessments, signpost to other services as and when required. The team is integrated with Social Workers, Occupational Therapists, Social Care Assessors, and Occupational Therapy Assistants. Assessments are mainly telephone based but with opportunities to carry out home visits when required.

You will have:

- NVQ Level 3 in Health and Social care or equivalent (Desirable).
- Be required to undertake an enhanced DBS plus Adults and Childrens Barred List (Essential).
- Experience in working directly or indirectly with the public
- An understanding of the principles and practises of care
- Excellent customer service skills
- Effective teamwork and organisational skills
- The ability to self-manage workloads within timescales for the completion of tasks
- You are confident, energetic, and enthusiastic as well as sensitive and resilient
- Effective interpersonal and communication skills including an excellent telephone manner
- The ability to manage difficult customer situations
