**Job Overview**:
Northern Healthcare provides mental health recovery services across many regions within the UK. Our services consist of Supported Living and community outreach services which provide our residents with access to 24-hour support from experienced clinical and support staff.

Currently our services are based across the Northwest, Cumbria, Yorkshire and Midlands supporting over 150 adults across the UK in all aspects of their daily lives and mental health recovery. We have developed a unique clinically led, recovery-focused support model that provides individuals with dedicated 24-hour support in high-quality accommodation.

We are looking to recruit an enthusiastic and motivated individual to join our team as a lead Assessor within a mental health and L&D setting. The post holder will provide clinically based assessments of adult with complex mental health needs of potential new resident for Northern Healthcare (NHC). They will also be required to undertake assessment of individuals with a learning disability and/or autism who will also be potential new residents of NHC. The post holder will be responsible for triaging referrals and providing feedback to the referrers.

**Key Responsibilities**:

- Lead on assessments of potential new residents for NHC
- Support the referral and assessment process by working collaboratively with the commercial and operational teams
- Develop transition, admission, and discharge plans, follow-up referrals and assessment for suitability for the services.
- To undertake assessments of individuals including those residents presenting with higher levels of risk.
- To support on-site teams with care and support planning and risk assessments of the residents that are new into service.
- To offer specialist clinical assessment and advice where required.
- To assess and manage ongoing risks as identified during the assessment, ensuring that this is done in collaboration with the resident and carers.
- To ensure that clinical practice is evidence-based and consistent with relevant guidelines and policies and procedures.
- To act within the relevant professional code of conduct at all times.

- Ability to offer comprehensive mental health assessments underpinned by evidence-based knowledge and collate information with a view to having a comprehensive case.
- Ability to formulate support plans, risk management plans formulate risk and be part of a wider team to assess risk.
- Out of hours on call for crisis management on a rotational basis.
- Referral and liaison to secondary care.
- To safeguard each resident from any form of abuse including; (Physical, psychological Financial, Sexual, Institutional, Unintentional abuse) from staff, visitors or each other and report any abuse observed immediately.
- To be a designated Safeguarding lead for NHC.
- To participate and lead the weekly escalation calls.
- Support with training needs related to mental health and/or learning disability diagnosis.
- Ensuring MCA principles and codes of practice are always followed and the legal framework to support Residents lacking mental capacity and the procedures that must be followed in such circumstances.
- Working knowledge of all relevant legislation associated with LD and mental health care, including MHCA and MHA.
- Ensure timely and accurate documentation and maintain confidentiality within legal, organisational and ethical requirements.
- Ability to manage high-risk and complex presentations in community with positive therapeutic risk
- Ability to undertake medication management, medication audits and complete medication competencies to care
- Understanding of the Mental Health Act (1983) and monitoring of required
- Utilise strategies to enhance Recovery for our residents.
- Ability to advocate on behalf of residents to support them to access a range of services that, as far as possible, reflect their needs and wishes.
- To promote awareness of the professional role within the team and across the service by running workshops or training sessions for staff where required.
