Job Description

We are seeking a highly skilled Mental Health Care Coordinator to join our team at Greater Manchester Mental Health.

This is a fantastic opportunity to work with a dynamic and dedicated team, providing high-quality care to individuals in need.

The successful candidate will have excellent communication skills, both written and verbal, and be able to work effectively with multidisciplinary teams.

Main Responsibilities:
  • Assess service users' physical health needs and provide appropriate support and guidance.
  • Work collaboratively with care coordinators to develop and implement personalized care plans.
  • Participate in multi-disciplinary team meetings to discuss service user care and treatment plans.
  • Contribute to the development and implementation of new services and programs aimed at improving mental health outcomes.
Requirements:
  • Recognized competency-based level 3 NVQ in a relevant subject area or equivalent demonstrable experience.
  • Experience of working in mental health care settings, preferably in a community setting.
  • Excellent organizational skills and ability to work flexibly.
Benefits:
  • A competitive salary of £23,300 - £38,700 per annum, depending on experience.
  • 27 days annual leave plus bank holidays, rising to 29 days after 5 years and 33 days after 10 years.
