The Gender Identity Clinic (GIC) is the largest and oldest gender clinic in the UK, dating back to 1966. We accept referrals from all over the UK for adults with issues related to gender. We accept referrals for patients who are aged 17 years and older onto our waiting list. Patients will not be offered an appointment before their 18th birthday.

We are a multi-disciplinary team, including psychologists, psychiatrists, endocrinologists, speech and language therapists, and nurses. We work together to provide holistic gender care, focusing on the biological/medical, psychological and social aspects of gender. The CNS (Core pathway) plays a vital role in assessing, monitoring and managing the physical and psychological wellbeing needs of the caseload of patients under the care of the Gender Identity Clinic. They will facilitate clinical sessions, assess / triage people according to agreed assessment tools, offering short- term interventions based on psychological therapies modalities and offer professional advice to patients and their carers, other professional, statutory and voluntary agencies.

The CNS will hold a continuous responsibility for a defined caseload in the service. Liaise between primary care and where indicated, secondary care mental health teams to ensure clear communication and seamless care pathways, thus enabling access to the appropriate level of care and intervention.

Main duties of the job

* Manage a caseload of patients whilst working as part of a multidisciplinary team (defined or virtual), delivering individualised and personalised direct patient care.

* Coordinate the triage referrals into the service and conduct clinical harm reviews for patients on the waiting list.

* Provide specialist nursing service and advice in a variety of clinical settings and disciplines for staff, patients, their families and carers, effectively liaising with multidisciplinary colleagues.

* Receive referrals via a variety of media including (where defined) direct patient referral. Triage case load according to agreed protocols and working practices.

* Carry out those activities required to conduct and record a comprehensive assessment of a person's nursing requirements. Be mindful and record relevant details of cognitive understanding, spiritual and religious beliefs, and patient's wishes.

* Coordinate the triage of referrals into the service, conduct clinical harm reviews for patients on the waiting list and review dormant cases.

* To effectively communicate complex information concerning the assessment, formulation, and treatment plans competently, and sensitively to patients under their care.

* Take part in a duty of distress Rota as required.

* To deliver nurse-led clinics under the supervision of an appropriate senior clinician in the clinic.

Qualifications

Essential


  • Registered Nurse with the NMC
  • Degree level education or equivalent
  • Evidence of post graduate education and training/CPD relevant to role

Desirable


  • Mentorship Qualification (or Practice Assessor trained)
  • Independent prescriber with experience of prescribing or willingness to undertake training

Experience

Essential


  • Experience of working with Gender Patients or mental health
  • Experience of employing psychological treatment modalities and or sexual health
  • Experience of supervising and/or managing junior staff
  • Experience of supporting/teaching/supervising nursing students or equivalent

Desirable


  • Experience of working in a Band 7 role
  • Experience of writing or contributing to research grants and/or publications

Skills

Essential


  • Clinical nursing skills and knowledge to measure, record and interpret observations and other relevant clinical information in the assessment and care planning process - including in the context of consultation with service users with comorbidities and complex presentations.
  • Evidence of skills and knowledge in advanced clinical assessment/physical examination or willingness to undertake this training.
  • Clinical documentation, record keeping and report writing skills Word processing and database skills.
  • Good time management, prioritisation, and organisational skills to balance competing demands, complete work in a timely fashion and meet deadlines.
  • Able to demonstrate ability to work with other disciplines.

Knowledge

Essential


  • Understanding of NMC Code of Conduct and Scope of Professional Practice
  • To act as a clinical expert in this specialist area of practice through high quality consultation, advice, support and direct intervention
  • Knowledge of adult and child safeguarding procedures and processes
  • Recognise signs of mental health distress and escalate or signpost as appropriate to ensure risk is assessed and managed appropriately
