Main duties of the job

As a Clinical Specialist Nurse you will provide nursing leadership, in particular supervising non qualified staff to support a large caseload of people living with Dementia and their care givers.

You will work alongside other lead practitioners to support staff and team development across Camden SAMH teams. You will support team members in the Memory Service through our own integrated MDT and in collaboration with external partners. You will hold your own caseload of older adults with Dementia who require complex mental, physical and social care interventions.

We are a busy service and caseloads are reflective of this. This requires you to have good organisational skills and an ability to work independently. The role involves visiting patients at home and you therefore must be able to get around Camden using Public Transport and/or walking/cycling.

You'll be able to demonstrate significant experience in working with older people and/or dementia care, excellent communication and interpersonal skills and clear leadership skills. Being computer literate is a must and you have to be able to use Word, Excel and other relevant IT systems.

You will also be responsible for supervising other members of the team

Job description

Job responsibilities

The post holder will be a Clinical Specialist Nurse within the Camden Memory Service for the Services for Ageing & Mental Health (SAMH).

Many service users also require support with the management of comorbid long-term conditions. The post holder will carry a caseload of complex service users in the service, in addition to providing nursing supervision and consultation to non qualified practitioners and other professionals. As such, the post holder will be required to have significant clinical skill and knowledge underpinned by experience, and specialist training / academic study.

The post holder will be required to be knowledgeable of current national policy and best practice guidelines in caring for older people diagnosed with dementia and will work closely with team managers ensure care is delivered in accordance with these.

The post holder will formulate and support others to formulate profession specific, highly specialist care and treatment plans that utilise health and social care resources to support the needs of service users with functional illness and frailty, dementia diagnoses, and co-morbid long-term conditions.

Education/Qualification

Essential


  • Registered Health Professional - Nursing (all registrations), OT, Social Work
  • Evidence of further post-registration relevant training /ongoing academic study relevant to area of practice
  • Other advanced relevant training i.e. Dementia Care/Advanced Care Planning etc

Desirable


  • Evidence of Level 2 Safeguarding Adults and Children

Knowledge and Experience

Essential


  • Extensive experience of working in a specialist capacity for people with dementia in a community setting
  • Demonstrate a clear understanding of the Mental Capacity Act legislation, its application and use in practice with people with dementia
  • Demonstrate practical experience of supervision within a clinical context
  • Demonstrate an understanding of risk together with an ability to evaluate and contain risk with reference to the individual, staff and carers
  • Evidence of participation in service development/Quality Improvement project

  • Excellent verbal and written communication skill
  • Able to work autonomously and to deputise for senior staff in a variety of roles when required to do so.
  • Ability to maintain boundaries and form positive therapeutic relationships with service users and carers
  • Competent using computers and keyboards for entering personally generated data, generating and responding to emails, conducting web searches, storing data and retrieving data, producing reports etc
  • Highly motivated and committed to improving quality and services for people affected by dementia
