An exciting opportunity has arisen for three newAdult Support Coordinators to join us within the Prevention and Early Intervention Services. One post will be based in the Adult Early Help Team and twoin our Assessment Team

Adult Support Co-ordinators play a major part of the work we do in our Teams. You'll be working with adults and older people in need of care and support services, along with their informal carers. It will be crucial that you work in partnership with them in order to establish needs and ensure they're met.

The Adult Early Help Team is the front door to Adult Social Care and look to provide advice, guidance and onward referrals to enable people to remain independent in their communities.

The Prevention and Early Intervention Assessment team provide support to those adults who have had a period of Reablement or Intermediate Care team support. We also provide Carers Assessments to those in informal caring roles who's cared for person is not in receipt of statutory services.

Our teams are multidisciplinary and offer support to individuals and also to families and carers. The team work closely to support and share information on prevention services across the county.

You won't need to have a social work qualification but knowledge, experience and understanding of the needs of vulnerable adults will be essential to succeed in the role. Working knowledge and experience of the Care Act, Mental Capacity Act and Cambridgeshire County Council's Adult Social Care policies and procedures will also be an advantage, particularly around wellbeing and outcomes.

What will you be doing?

You'll be working with individuals and their informal carers to complete Community Action Plans, Care Act assessments, care and support plans, and carers assessments, initially reviews alongside having a role in Safeguarding and considering capacity.

Inquisitive and creative practice is embraced, to allow for personalised solutions, taking into account wider community resources and organisations. The work will be ensuring timely discharge from the Reablement and Intermediate Care teams across Cambridgeshire, working closely with social care and health colleagues to ensure correct discharge plans are in place.

About you

You will need to have good and accurate literacy, numeracy and administrative skills and the ability to analyse information to solve general queries. It is imperative that you have strong communication skills, a confident telephone manner and can work on your own initiative. Good planning and organisation skills will be key, and you should be used to working under pressure to meet deadlines, allowing flexibility and resilience. You will need a good working knowledge of the Care Act 2014 and Mental Capacity Act 2005. Experience of multi-disciplinary working to enable best outcomes for individuals.
