Taunton and West Somerset Neighbourhood OPMH would like to welcome an Older Person's Mental Health Practitioner to our team.

The team provides needs-led assessment and treatment and support, for older-aged adults in the community, working closely with the I npatient Ward, First Response Service, Intensive Dementia Service other CMHTs, GPs, Primary Care Network MDTs and Voluntary Services . We work extremely closely with the rest of the integrated neighbourhood teams, which include integrated rehabilitation team and district nursing.

The role will involve managing a defined caseload with a focus on working with individuals and their significant others who experience a complex range of mental health needs and difficulties. Acting as a key member of the Multi Disciplinary Team working within an Integrated Service.

Main duties of the job

You'll undertake complex mental health assessments, care co-ordinate a caseload, have access to psychosocial support, be collaborative in their outlook and must be skilled in positive risk management whilst working within a recovery approach. We work very closely with a wide range of agencies in our community and will be central to this fulfilling role.

You will be committed to a person-centred approach to your work and will have an ability to prioritise and work flexibly. You will hold a caseload that will include patients with functional and organic diagnoses. You will also be part of the Duty worker system that is in place but always be supported by a duty manager and your fellow colleagues - we foster a true team approach

The team's hubs are based in Taunton and Minehead and whilst you will be based in Minehead there may be occasions when you will be expected to work across both areas to cover the Taunton and West Somerset Neighbourhood.
