The Deputy District Nurse Team Leader is responsible for the clinical leadership and management of a geographical caseload. This would include the holistic assessment of a patient care needs and development, implementation and evaluation of evidence care.

The post holder will participate in the delivery of skilled, autonomous nursing care to patients within their own home. Duties include palliative care support and assessment, care at the end of life, supporting and assessing individuals and carers with long-term health conditions, promotion of self-management skills and wound care.

Main duties of the job

The post holder will be responsible for providing support to the Team leader in the day to day management of multidisciplinary/multiagency packages of scheduled and unscheduled care for patients with multiple, complex and palliative care needs. Assisting the Team Leader in the requirements of the nursing team by utilising effective management and leadership skills.

Responsible for the delegation of clinical nursing activities to appropriately skilled members of the community nursing team, ensuring that an efficient and effective service is delivered to the practice population.

The post-holder must be a Registered Nurse and hold a Diploma/Degree/Masters in Community Health Studies (District Nursing) and be a registered Nurse Prescriber.

Qualifications and/or Knowledge

Essential


  • Live registration with NMC as Registered Nurse
  • Post registration District Nursing qualification (BSC in relevant subject)
  • Experience of proactive caseload management of wide range of patients including those with chronic conditions

Desirable


  • Independent prescribing qualification
  • MSc Advanced Clinical Practice
  • Management / Leadership qualification or equivalent preparation
  • Experience at management level demonstrating ability to deliver improvements to service users' care

Experience

  • Significant clinical experience in assessing, planning, implementing and evaluating care for the patient group of the caseload
