Main duties of the job

This is an ideal opportunity for a Health Care Support Worker who is seeking to broaden and utilize their range of skills and knowledge to nurse people in their own environment as part of a multi-disciplinary team. The post holder will participate in the delivery of skilled, autonomous nursing care to patients within their own home.

The Crickhowell District Nursing Team are an integral part of the Virtual Ward that aims to prevent unnecessary admissions to hospital and facilitate early discharge home.

This postinvolves lone working and the ability to travel independently is essential.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Qualifications and/or Knowledge

Essential

NVQ3 in Health or Social Care or equivalent demonstrable experience in a Health or Social Care setting

Desirable

ECDL or equivalent level of computer skills To be aware of the needs of the patients and their relatives/carers and have knowledge of voluntary and statutory services

Aptitudes & Abilities

Essential

Meets all essential criteria Can demonstrate good interpersonal and communication skills both written and verbal Ability to work within a multidisciplinary team Ability to manage stressful situations in self Ability to travel between sites in a timely manner Can explain the importance of care monitoring, audit and evaluation of fundamentals of care

Desirable

Ability to speak Welsh Good time management and ability to prioritise Ability to work alone under the supervision of Clinical Lead Able to undertake extended tasks as required by the Clinical Lead
