The Deputy District Nurse Team Leader is responsible for the clinical leadership and management of a geographical caseload. This would include the holistic assessment of a patient care needs and development, implementation and evaluation of evidence care.


Providing safe, effective, harm-free care
Working within the community resource team and industry partners for the benefit of patients and the population
A questioning, analytical, empowered Nursing and Midwifery workforce

The post holder will participate in the delivery of skilled, autonomous nursing care to patients within their own home. Duties include palliative care support and assessment, care at the end of life, supporting and assessing individuals and carers with long-term health conditions, promotion of self-management skills and wound care.
The post holder will be responsible for providing support to the Team leader in the day to day management of multidisciplinary/multiagency packages of scheduled and unscheduled care for patients with multiple, complex and palliative care needs. Assisting the Team Leader in the requirements of the nursing team by utilising effective management and leadership skills.


Responsible for the delegation of clinical nursing activities to appropriately skilled members of the community nursing team, ensuring that an efficient and effective service is delivered to the practice population.

The post-holder must be a Registered Nurse and hold a Diploma/Degree/Masters in Community Health Studies (District Nursing) and be a registered Nurse Prescriber.

Live registration with NMC as Registered Nurse
Post registration District Nursing qualification (BSC in relevant subject)
Experience of proactive caseload management of wide range of patients including those with chronic conditions

Management / Leadership qualification or equivalent preparation
Experience at management level demonstrating ability to deliver improvements to service users' care

Significant clinical experience in assessing, planning, implementing and evaluating care for the patient group of the caseload


Ability to use computer systems to a well-developed level (email, word processing, spreadsheets)
Demonstrable ability and track record of acting with care, compassion, promoting dignity and respect, role modelling the highest standard of professionalism
Knowledge of current Nursing issues within NHS care systems
High level of knowledge of NMC code of conduct and professional standards/guidance
