Black Country ICB has an exciting opportunity for two new Heads of Service, Continuing Healthcare (CHC) to become part of the senior management team structure.

This role will support the Chief Nursing Officer and Deputy Chief Nursing Officer in the development and implementation of the strategic vision for the delivery of CHC across the Black Country system.

We are looking for someone that is innovative and has strong leadership skills to inspire our CHC Teams across the place and system teams. Ensuring the provision of safe, high quality and effective care to all people that require funded support at local place and contribute to the strategic development of CHC across the Black Country system.

The role will be responsible for the management and leadership of local place-based and system based CHC Teams and will lead on transformation, assurance and case management. Working in a matrix management arrangement you will work to inspire and lead the local place-based team, working with statutory partners, local authority, and health providers to ensure ICB statutory duties are delivered for CHC assessment and funding.

Applicants must be an experienced manager with extensive knowledge of CHC and statutory requirements for ICBs. The role requires experience in establishing & developing relationships between commissioning organisations, commissioned service providers & ensure all relevant contractual requirements are fully applied.

Main duties of the job
  • Provide leadership and overall management expertise to the service, driving change and improvement by supporting and influencing the team and the corporate management team
  • Ensure that the ICB meets their statutory requirements in relation to provision of NHS Continuing Healthcare and other legislation in regard to Complex Care (e.g. Mental Health Act and DoLS).
  • The post holder will be required to achieve against a range of objectives and priorities related to the overall management of the patient pathway. Performance will be monitored via the ICB Quality & Performance Framework and against action plans developed in response to requests from ICBs and from external regulatory bodies.
  • The role has the overview of the NHS continuing healthcare service across the local place and is responsible for promoting and delivering a consistent approach, manage the changes in process and improvement in service delivery, working in partnership with arange of external partners and providers to ensure the delivery of a high quality CHC service.
  • Oversight and responsibility for commissioning and approval of new services for Continuing Health acre (CHC), Discharge to Assess (D2A) and Personal Health Budgets (PHB).
  • Oversight and management of CHC appeals process and adherence to required timescales and manage contentious situations in relation to disputes between partner agencies or with individual patients and relatives.

Job responsibilities

  • Act as a visible and accountable leader within the local healthand social care economy to promote good practice in the Continuing Healthcare,Discharge to Assess and Personal Health Budget pathway across partnerorganisations including timely access and decision-making
  • To be the expert lead on NHS Continuing Healthcare at the place,giving advice and input where requested.
  • Work collaboratively across the wider ICB with Heads of CHCservice at place and also with key stakeholdersincluding, but not exclusively, Local Authority, NHS Trusts, third partyproviders, service user and carers groups
  • To hold lead responsibility for ensuring that all processesimplemented are compliant with legislative andnational guidance and in accordance with the responsibilities of the ICB.
  • Ensurecompliance with best practice guidelines, risks are proactively managed andissues identified, investigated and resolved.
  • Supportthe Chief Nursing Officer by contributing to the strategic planning of the CHCfunction within the ICB, as well as providing technical advice to improvecapacity planning and delivery
  • Supportproductive and positive whole systems working in order to ensure serviceresources are deployed effectively to deliver a responsive, high quality and anexcellent patient experience
  • Evaluateperformance of CHC and Complex Care assessments to ensure quality, consistencyand maintenance of best practice within changing requirements.
  • Ensuretraining is established for all Continuing Healthcare staff to achieve on-goingimprovements in the system and keep abreast and disseminate policychanges.
  • Representthe place, ICB at regional and national conferences and on external workinggroups as appropriate
  • Tocreate a vision for the CHC service that is aligned to the delivery of the ICBcorporate objectives and which links with the overall strategy forservices
  • Ensuringthe strategic development of a patient pathway which is safe, efficient,effective and economic, and which contributes fully to the ICB priorities andtargets
  • Overallresponsibility for the delivery of agreed performance metrics as required bythe Chief Nursing Officer and to support the delivery of key nationaltargets
  • Authorisingof individual care packages for Continuing Healthcare eligible patients up tothe delegated authorisation. Ensuring that oversight of the budget ismaintained, data and trends are identified, solutions and actions are agreed toensure that financial balance is achieved
  • Authorisationfor specialist health equipment for non CHC patients within delegated budget
  • Responsiblefor the budget for Continuing Healthcare, Funded Nursing Care and Complex Care.This will include participation in budget setting, procurement and overallbudget management.
  • Providein depth analysis, interpretation and production of complex and multiplereports including financial returns internally and external organisations(NHSEI) in relation to performance, quality and activity
  • Accountablefor ensuring there is a plan in place to improve staff engagement andsatisfaction for line managed staff with a focus on improvement of staff surveyresults
  • Provideboth strategic thinking and technical advice to develop and implementinnovative capacity plans and improved processes
  • Toencourage service user/carer feedback on the service provided and toimprove/develop the service in response to this feedback
  • Leadon the development of quality, performance and governance strategies and thedevelopment and implementation of improvement programmes, in accordance withnational and local priorities

Person Specification Qualifications
  • NMC Professional registration
  • Educated to Masters degree level or equivalent level of experience of working at a senior level in continuing healthcare speciality or other complex health or social care service
  • Evidence of post qualifying and continuing professional development
  • High level understanding of the legislation, national policy and issues as they relate to NHS Continuing Healthcare
Experience
  • Significant demonstrable experience with line management responsibility at a senior level
  • Proven success of change management, including ability to motivate staff
  • Previous experience in budget management
  • Evidence of clear understanding and experience of safeguarding process, Mental Health Act & Mental Capacity Act
  • Evidence of broad experience of cross sector partnership working
Skills and Knowledge
  • Awareness of the broad strategic direction of the organisation
  • Extensive working knowledge of National Framework for NHS Continuing Healthcare and Funded Nursing care and its application in practice
  • Excellent verbal and written communication skills Highly developed negotiating, facilitation and influencing skills
  • Excellent writing skills, including the ability to summarise information and key issues
  • Ability to collaborate constructively with internal and external partners to create the conditions for successful partnership working
  • Experience of dealing with complex and sensitive information and unpredictable situations
  • Ability to identify risks, anticipate issues and create solutions and to resolve problems in relation to service delivery
  • Ability to function in extremely stressful situations, assimilating a variety of clinical and managerial information
  • IT literate with good working knowledge of Microsoft Office particularly Outlook, Word and Excel with standard keyboard skills
