Do you strive or aspire to provide seamless holistic patient centred care within a progressive
Street Community Hospital. The team works across the Durham, Chester-le-Street and Derwentside area providing therapy into rehabilitation beds within care homes and out into the community. The post involves working as part of multi-disciplinary team including Occupational Therapists, Physiotherapists, Therapy Assistant Practitioners and Assistants.
The successful applicant will independently manage a caseload in the intermediate care beds and into the community providing holistic, short term, goal orientated interventions. Patients will present with a wide range of complex problems and will allow you to develop a highly specialist knowledge of many medical conditions and long term conditions and the application of the physiotherapy process with these patients.
Progress skills in holistic assessment and providing comprehensive rehabilitation plans
Develop skills around discharge planning and equipment provision
To support assistant staff and to help with their development
Participate in service development, peer support and in-service training
The successful applicant would work Monday to Friday in the Intermediate Care Bed service. We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Complete holistic and specialist assessments and create specialist rehabilitation plans
To delegate treatment plans to unqualified staff inclusive of assistants and care support workers.
To contribute to the supervision of undergraduate therapists where placements within speciality areas have been designated as appropriate.
To contribute towards the provision of flexible and timely services, ensuring that assessments and treatments are carried out in a timely fashion as per team operation guidelines.
To work to the standards of the speciality area and to ensure that expert knowledge within the speciality area is acquired through independent learning and collaboration during the working day with senior staff.
To attend mandatory training and to adhere to all Trust policies and procedures as they pertain to personnel, health and safety, child / elder protection issues and continuing health care needs.
BSc Physiotherapy or equivalent
Continuing Professional Development Diary including evidence of reflective practice, training and clinical experience

Recognised relevant academic qualification
Relevant experience in physical rehabilitation including recent general rehabilitation
An understanding of legal responsibilities within the profession
Ability to travel independently across Trust sites and the locality, and carry small items of equipment on a daily basis to fulfil the requirements of the post
